Why did the optometrist dilate my eyes, and can you drive after?

The drops sting slightly for a second. Twenty minutes later the room is too bright, your phone will not come into focus, and everything close up has gone soft. If nobody explained why, it is unsettling. Here is what is happening.

What the drops do

Dilating drops relax the muscles that control your pupil, so it opens wide and stays open. They also temporarily reduce your eye's ability to focus up close, which is why near vision blurs rather than distance vision.

Both effects are temporary and both are the point. Nothing is being done to your eye beyond making it briefly hold a wider aperture.

Why a wider pupil matters

Your pupil is a window. Looking at the retina through an undilated pupil is like inspecting a room through a keyhole — you can see the middle of it, and you cannot see the corners.

The corners are where a lot of important things live. The peripheral retina is where tears and detachments tend to begin. Early diabetic changes in the small blood vessels show up across the whole retina, not only in the centre. A wide pupil turns the keyhole into an open door, and lets the examiner see the whole surface rather than a circle in the middle of it.

This is why dilation is not routine for everyone at every visit, and is strongly indicated for some people: anyone with diabetes, anyone with a high short-sighted prescription, anyone reporting new floaters or flashes, anyone with a family history of retinal disease, and generally anyone older where the yield from a full retinal view is higher.

How long it lasts, and what it feels like

Expect several hours. For most people the bulk of it passes within about four to six hours, and some notice mild light sensitivity into the evening. Lighter-coloured eyes sometimes stay dilated longer.

During that window: bright light will be genuinely uncomfortable, because your pupil cannot constrict to protect you from it. Near work will be difficult — reading, phones, anything detailed. Distance vision is usually much less affected, which fools people into thinking they are fine.

Can you drive after?

Plan on no. Not because the law necessarily forbids it, but because you will be sensitive to glare and unable to focus on your dashboard or phone, and daytime sun in the Philippines is exactly the condition dilation handles worst. Oncoming headlights are worse again.

Arrange it properly:

• Bring someone who can drive, or plan to book a ride
• Bring sunglasses, and if you do not own a pair, ask at the counter for the disposable shades
• Book a morning appointment rather than late afternoon, so the worst of it passes in daylight rather than on a night drive
• Do not schedule a meeting, an exam or anything requiring reading straight afterward

When to call rather than wait it out

Mild stinging on instillation and hours of blur and light sensitivity are expected. Contact the practice or an eye doctor promptly if you get severe eye pain, a red eye with a hard ache, vision that is getting worse rather than gradually better, or nausea and vomiting with eye pain. These are uncommon, and they are not something to sleep on.
