High blood pressure and your eyes: what an eye test can see

Hypertension is common in the Philippines and it is famously quiet — many people carry it for years without a symptom. Which is why one fact about the eye is worth knowing: the retina at the back of your eye is the only place in the body where a clinician can look directly at small blood vessels without cutting anything open.

Everywhere else, the state of your circulation has to be inferred. In your eye, it can be seen.

What high blood pressure does to eye blood vessels

Sustained high pressure changes small arteries over time. The walls thicken and narrow. Where an artery crosses a vein, it can press on it. In more advanced cases, vessel walls leak, producing small haemorrhages, or deposits and swelling appear in the retinal tissue. This group of changes is called hypertensive retinopathy.

The important point for a reader: these changes usually develop without affecting your vision at all in the early stages. You will not notice them. Someone looking at your retina will.

The related risks

High blood pressure also raises the risk of blockages in the blood vessels serving the eye. A blocked retinal vein or artery can cause sudden, painless vision loss in one eye. Those events are emergencies — not because glasses can help, but because the cause needs urgent medical attention.

Hypertension and diabetes together are more concerning than either alone, because both damage the same small vessels. If you have both, eye monitoring is not optional maintenance.

Symptoms to take seriously

Get medical attention urgently if you have any of these, particularly if you know your blood pressure runs high:

• Sudden loss or dimming of vision in one eye, painless
• A shadow or curtain across part of your vision
• Sudden double vision
• Severe headache with visual disturbance
• A sudden shower of new floaters, or flashes of light

A burst blood vessel on the white of the eye, by contrast — a bright red patch that looks alarming and does not affect vision — is usually harmless and settles on its own. It is worth mentioning at a check-up if it keeps happening, but it is not an emergency by itself.

What an eye test can and cannot do

Be clear about the limits. An eye examination does not diagnose or treat high blood pressure, and it does not replace seeing your doctor or taking your medication. Your blood pressure is managed by your physician. What an eye test adds is a direct view of what that pressure has been doing to your small vessels over time — information that is genuinely useful to the doctor managing you.

It also works the other way around. Vessel changes are sometimes noticed at a routine eye test in someone who did not know their blood pressure was high. That is not a rare event.

How often to get checked

If you have been diagnosed with hypertension, ask your physician how often they want your eyes examined — it depends on how well controlled your pressure is and what else you have. As a general habit, adults benefit from a comprehensive eye exam regularly rather than only when vision changes, because the things worth catching early are the ones that do not announce themselves.

Bring your medication list to the appointment. Some medicines affect the eyes, and the optometrist should know what you are taking.
