Can astigmatism get worse over time?

Yes, it can change — and it can also improve slightly. For most people the change is slow and small, measured over years rather than months. What matters is not whether the number moved, but how fast it moved and why.

What astigmatism actually is

A perfectly shaped cornea is like the surface of a basketball — curved the same in every direction. An astigmatic cornea is shaped more like the back of a spoon: curved more steeply one way than the other. Light entering the eye focuses at two different points instead of one, so everything is slightly smeared rather than simply blurred.

That is why astigmatism blur feels different from short-sightedness. Letters look doubled or shadowed. Point lights at night stretch into streaks. Fine print takes effort even at a comfortable distance.

On your prescription it appears as two numbers: CYL, which is how much astigmatism there is, and AXIS, which is the orientation of the steeper curve in degrees. Both have to be right for the lens to work.

What makes it change

• Childhood and adolescence. The eye is still growing. Astigmatism commonly shifts alongside myopia during school years.
• Normal ageing. The orientation of astigmatism tends to drift gradually through adult life as the cornea and lens change shape. Slow drift is expected.
• Changes in the crystalline lens. As the lens inside the eye ages, it can add its own astigmatic component. This is one reason a prescription can move more noticeably after the fifties.
• Eye surgery or injury. Anything that changes corneal shape changes astigmatism.

Reading in dim light does not cause astigmatism. Neither does wearing your glasses too much, or not enough. Those are two of the most persistent beliefs we hear across the counter, and neither holds up.

When a change is worth investigating

A fast increase in astigmatism, particularly in a teenager or young adult, is worth a proper look. Rapid, asymmetric steepening can be a sign of keratoconus — a condition where the cornea progressively thins and bulges. It is not common, but it is far more manageable when it is caught early, so it is the thing an optometrist wants to rule out rather than assume.

Signs that deserve a same-month appointment:

• Your astigmatism has jumped noticeably at consecutive checks.
• New glasses stopped working within months.
• Increasing ghosting, halos or streaking at night.
• Frequent eye rubbing, especially with allergies — habitual hard rubbing is associated with corneal changes.

None of this is a diagnosis. It is a reason to get measured properly rather than to keep updating your prescription online.
